What is endoscopy in fertility care?

In endoscopy, a very narrow scope with a camera is used for visualizing the internal structures either via natural openings or small incisions. Hysteroscopy, an examination and treatment of the uterine cavity without making any cuts, and laparoscopy, which is the examination and treatment of the pelvic organs with small incisions, are the two important types of endoscopies performed during the process of infertility management. The reason why these techniques play an important role in infertility surgery is that these techniques provide accurate diagnosis, treatment, and faster healing than open surgical operations. Endoscopy operations are generally done as day-care or short stay.

Assessment and planning

Non-invasive tests first, so the endoscopy answers a specific question. Pre-operative bloods, a pre-anaesthetic check, fasting from midnight, and an escort home.

The procedure

Hysteroscopy through the cervix, laparoscopy through small abdominal incisions, or both in one anaesthetic. Where pathology is found it can usually be treated in the same sitting.

Recovery and findings

Home the same day in most cases. Recovery is one to two days after hysteroscopy alone, three to seven after laparoscopy. Findings and any treatment are explained before you leave.

What to expect

When endoscopy is not the right step

  • Non-invasive tests have not yet been completed
  • Pregnancy is possible, or there is an active pelvic infection
  • IVF is already planned and the findings would not change it
  • Where nothing that could be found would alter the treatment plan
  • Where surgical and anaesthetic risk outweighs the likely benefit

Endoscopy is surgery. It is worth doing when the answer changes the plan, and not otherwise.

What to expect afterwards

  • Home the same day in most cases
  • Cramping and light spotting after hysteroscopy
  • Shoulder-tip discomfort and abdominal soreness after laparoscopy
  • One to two days off after hysteroscopy; three to seven after laparoscopy
  • Combining both in one anaesthetic spares you a second procedure
